A primeira descrição dessa síndrome foi apresentada por Leo Kanner, em 1943, com base em onze casos de crianças que ele acompanhava e que possuíam algumas características em comum: incapacidade de se relacionarem com outras pessoas; severos distúrbios de linguagem (sendo esta pouco comunicativa) e uma preocupação obsessiva pelo que é imutável (BOSA e CALLIAS). Esse conjunto de características foi denominado por ele de autismo infantil precoce (Kanner, 1943).
O diagnóstico e subclassificações do Autismo estiveram sob o rótulo de ‘esquizofrenia infantil’ por muitas décadas. Entretanto, segundo Rutter (1985), já havia nos anos 70, um reconhecimento de que seria necessário distinguir-se entre as desordens mentais surgidas na infância, e as psicoses cujo aparecimento se faz mais tarde. Considerando que uma séria anormalidade no processo de desenvolvimento está presente desde cedo na criança, o termo ‘transtornos invasivos do desenvolvimento’ tem sido adotado, desde a década de 80. O estudo do autismo, desde as primeiras considerações feitas por Kanner (1943) até as mais recentes reformulações em termos de classificação e compreensão dessa síndrome (Rutter, 1996), tem sido permeado por controvérsias quanto a sua etiologia. Historicamente, reivindicações a respeito da natureza do déficit considerado ‘primário’ (inato x ambiental) têm constituído os principais postulados das teorias psicológicas sobre o autismo. Têm sido propostas teorias psicanalíticas, afetivas, sócio-cognitivas, neuropsicológicas e de processamento da informação.
